Sample Letter to refund amount :mobile recharge

Sir,

 I have recharged BSNL mobile number

 with Rs 99

 from my sbi internet banking account through oxi/paytm services on 
19​.01.2015 .

The problem is the amount of Rs 99 has been deducted from my bank account but i have not received the recharged amount to my mobile number.

 So please refund the amount to  my bank amount.
